Congratulations on the birth or adoption of your child. We welcome you with great joy as you seek baptism for your child in our parish community.

Please submit this form AT LEAST TWO (2) WEEKS BEFORE the Baptismal date. Please complete this form accurately as it will be used for recording your child's baptism.

The Collaborative celebrates the Sacrament of Baptism at 1:30pm once a month at St. Edward and St. Jude parishes.
